Provide pharmaceutical care to patients with an emphasis on clinical monitoring and therapeutic interventions that improve patient outcomes. Provide information and educational programs to customers. Assist with the maintenance of performance improvement processes to constantly improve medication‑use processes. Assure dispensing accuracy and direct pharmacy workflow to ensure accurate and timely dispensing and distribution.
Job Duties- Medication Order Verification:
Review and interpret medication orders from licensed prescribers or their agents, ensure completeness and appropriateness, assess drug interactions, allergy alerts, duplication of therapy, and potential therapeutic interactions. - Therapeutic Clinical Monitoring:
Actively participate in monitoring medication therapy, direct therapy within collaborative agreements, and document assessments and recommendations in the patient chart. - Compounds and Dispenses Pharmaceuticals:
Ensure timely dispensing, supervise compounding per USP standards, verify labeling, use barcode technology, and supervise repackaging. - Supervises and Directs Interns and Technicians:
Provide direct supervision and mentoring, monitor task completion, encourage the use of the AIDET model, and report any non‑responsive behavior to leadership. - Medication Use Safety Initiatives:
Detect and report variances, participate in departmental efforts to increase safe medication use, and report suspected adverse drug reactions. - Controlled Substance Accountability:
Participate in receipt, dispensing, and distribution of controlled substances, reconcile inventories, and assist in resolving discrepancies. - Contribute to the positive overall financial status of the department:
Encourage formulary compliance, process non‑formulary requests, and participate in IV to PO conversions. - Drug Information / Staff Development:
Provide accurate drug information, serve as a preceptor, and develop staff education programs. - Maintain professional competency:
Maintain licensure, complete competency assessments, and keep current knowledge of pharmacy practice. - Perform other duties as assigned.
- Graduate from an ACPE‑accredited pharmacy college.
- Current US pharmacist license in the state where work is performed.
- Problem‑solving and critical‑thinking skills.
- Good communication skills.
- Familiarity with current practice standards (ASHP).
- Previous hospital experience.
- Non‑Exempt FLSA status.
