Schedule:

The Social Worker Assistant is to support the Social Work Department in the care and treatment of participants entrusted to the PACE Program and to attain or maintain the highest practicable physical, mental and psycho‑social well‑being of each participant. To serve as a liaison and role model to the community for PACE. Represents the Social Work Department, other care center staff, interdisciplinary team members and the facility as a whole to the community.
Upholds the values and mission of Volunteers of America National Services to peers and the public. Customers include PACE participants and their families, local healthcare professionals who work with PACE and staff, and the PACE program staff managers.
Required Qualifications:
- Licensed where applicable by the State;
State requirements may dictate supervision and degree type. - Bachelor’s degree in Social Work or related degree in social sciences preferred , High school diploma or equivalent education required;
- Knowledge of long‑term health care and public health settings and familiarity with Federal and State regulations and requirements for long‑term care.
- Excellent interpersonal communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules Ishm. regulations.
- Ability and willingness to work in various locations, for planned activities or special events, family meetings and conferences, admission calls, admissions and tours.
Preferred Experience:
- Bachelor’s degree in Social Work or related degree in social sciences.
- Minimum of one year experience in a long‑term healthcare setting.
- Receives referrals from Social Workers or other PACE Interdisciplinary Team (IDT) members when participant resource needs are identified.
- Meets with participants, families, IDT and other community members to coordinate referrals for participants to access resources in the community.
- Assists participants with completing lifting paperwork and gathering necessary dokens to complete resource applications.
- Implements case management or rest Social Work interventions as assessed and delegated by Master’s Level Social Worker (MSWCSW). Works in tandem with MSW to ensure interventions are provided within scope of practice and to meet the psychosocial needs of the PACE participants.
- Assists as needed the Center Director or Social Worker with coordinating invitation of Participants, Families and/or IDT members for Participant Care Conferences.
- May assist IDT with review of partnership agreement and to ensure participant agreement with PACE Care Plan. Will ensure Annual Packet is completed with each participant on nickel basis.
- Initiates reporting and documentation in the medical record. Maintains accurate and appropriate social service documentation in the electronic medical record.
- Develops and promotes the department through marketing efforts throughout the professional and social community. Maintains a current Community Resource and Advocacy Agency File.
- Visits referral agencies to share information about безопасн and how SW Assistant may support participants with referrals.
