Pay Range: $90,000 - $115,000

About the Job: The PACE Director of Clinical Services provides direction, coordination, supervision and implementation of professional and supportive services to residents/clients using nursing theory and process in accordance with the Kentucky RN Scope of Practice. The Director of Clinical Services assures the Senior Community Care PACE participants are treated with respect and dignity, recognizing individual needs and encouraging independence. The position will perform initial and ongoing assessment of physical & psychosocial needs, collaborate and coordinate with collaborate with other members of the PACE Interdisciplinary Team, healthcare system and and agencies and care providers as needed.
The position will oversee the care, counsel and health teaching of the ill, injured, or infirm and maintenance of health and/or prevention of illness of PACE participants.
Required Qualifications:
- Strong ability to create, build and maintain relationships at all levels.
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- Education:
Graduate of a school of professional nursing program required with Bachelors of Science Registered Nursing (BSN). Current State Registered Nurses License and Cardio Pulmonary Resuscitation (CPR) certification required
. - Experience: A minimum of four (4) years’ experience in working with the frail or elderly population required. At least four (4) years of nurse management experience in a hospital, nursing home or community-based setting is strongly preferred. Proven experience leading a team of professionals.
- Provides oversight and direction of the overall operational and clinical effectiveness of the PACE clinic, homecare department, and medication department.
