About the Role
Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long‑term care settings. In this role, you will be responsible for a panel of enrolled members receiving nursing home care. You will be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members.
As part of your role, you will conduct comprehensive assessments and develop individualized care plans for each member. You will actively participate in nursing facility care team meetings, maintain frequent communication with members’ families and loved ones, and ensure the care team stays aligned with members’ goals and needs.
- Provide collaborative primary care and services to a panel of Longevity Health Services, treating in the most appropriate setting based on the member’s goals of care.
- Execute and deliver contracted plan’s Plan Model of Care activities, including advanced care planning and the treating‑in‑place model.
- Facilitate, administer Health Plan clinical operations functions—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, promoting membership growth.
- Serve as a trusted advisor to members, loved ones and nursing home staff; educate care teams on member needs and early identification of changes in condition, notifying APP and telehealth partners for member changes.
- Deliver effective and frequent communication and coordination with members’ PCPs, loved ones and care team members on care needs, goals of care, and treatment plans/changes.
- Support overall market goals/targets and balance member caseload demands with team member support needs.
- Promote Longevity Health with members, loved ones and nursing facilities and other stakeholders.
- Function autonomously and be accountable for practice.
- Establish meaningful relationships and function effectively in the nursing home environment.
- Be member‑centric and customer‑service oriented, establishing meaningful relationships that ensure effective and high‑quality care for Longevity Health members.
- Perform other related duties as assigned.
- There are no supervisory responsibilities for this position.
- State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner; or state unrestricted licensed Physician’s Assistant, NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
- Active DEA License registered in the state of practice.
- Active national provider number (NPI) and active Medicare provider number.
- Eligibility to participate in the Medicare program as a provider and be registered.
- Valid state driver’s license with a good driving record and proof of automobile insurance.
- Willingness to drive up to one hour to and from work daily.
- Must be present in SNF communities/buildings during business hours daily.
- Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
- Minimum of 6 months experience as a Nurse Practitioner/Physician Assistant with some experience with geriatric/hospice or palliative care population prior to becoming an NP/PA, OR a licensed and board‑certified new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for 6 months and successfully completing all competencies for independent practice as an LHP APP.
- Experience working in the nursing home environment is strongly desired.
- Computer/technology proficiency required.
- Excellent people skills—ability to engage and communicate effectively with physicians, patients and family members.
- Self‑directed but functions well in a team environment.
- Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
- Demonstrated critical thinking, organization and prioritization skills.
