Opportunity to work as a Physical Therapist in a Part‑Time Home Health setting in Florence, OR. This role supports patient care in their homes, engages family and caregivers, and collaborates with an interdisciplinary team.
Job SummaryResponsible for patient evaluation, treatment planning, and care delivery within the scope of Physical Therapy. May provide clinical oversight and mentoring to assistants and students.
What you will do- Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation, and team members.
- Provides patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate.
- Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
- Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.
- May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students.
- Recommends and participates in development of new services and programs, quality improvement and monitoring activities.
- May participate in staff meetings, patient care meetings, educational in‑services, caregiver orientation and other professional activities.
- Makes recommendations for relative content and scope of therapy services and implements policies and procedures that guide and support the provision of services.
- Recommends and develops new therapy services and programs in collaboration with staff.
- May spend minimal time to assist with evaluating and orienting new caregivers and making recommendations to management regarding equipment and space needs related to physical therapy functions.
- Performs other duties as assigned.
- Bachelor's, Master's or Doctorate Degree in Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted).
- Physical Therapist License in the state of Oregon.
- One year of Physical Therapist experience, with home health/geriatric experience preferred.
- Upon Hire Basic Life Support through American Heart Association or American Red Cross.
- Valid driver’s license and proof of auto insurance.
Relocation and hiring bonus may be available.
CompensationSalary range: $47.96–$67.70 per hour, commensurate with experience. Part‑Time 0.80 FTE, Day position.
