Overview

Peace Health Rehab & Wellness Center in Florence, Oregon is seeking a Physical Therapist who will split time between Inpatient and Outpatient for a Full Time, 0.90 FTE, Day position. Salary range: $51.55 - $72.78. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Relocation and hiring bonus may be available.

About the team and environment

Join our team of therapists in a supportive and collaborative setting that values integrity and best-practice patient care. Our staff is supported by the organization with continuing education and knowledge sharing across clinics within Oregon. We deliver rehabilitation services to a diverse outpatient caseload and provide inpatient care in our 21-bed Critical Access Hospital. A Swing bed rehab program exists within the hospital, served by a dynamic interdisciplinary team.

The spacious remodeled gym features newer equipment, including electric mat tables, powered parallel bars, and Sci-fit bikes. We also have a dynamic vestibular program using Real Eyes xDVR Video Goggles. Each therapist has a computer-on-wheels to document at the point of service. Florence offers outdoor lifestyle opportunities with beaches, dunes, lakes and hiking nearby.

Job Summary

Responsible for the evaluation of patients, determining plan of care and goals of treatment, and participating in the treatment of patients within the scope of Physical Therapy. May provide supervision to therapy assistants and/or students following the plan-of-care and treatment of patients appropriate to the age of the patient served; may supervise therapy aides and/or volunteers. Works as part of a patient care team toward the best outcomes for the patient.

What

you will do
  • Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation, and team members.
  • Provides patient treatment according to the plan of care and modifies the plan to meet patient needs, updating the plan as appropriate.
  • Completes documentation accurately and in a timely manner, in accordance with regulatory and state licensure requirements and professional standards.
  • Collaborates with other healthcare professionals to ensure smooth continuous care of patients; may act as a liaison with nursing, medical and other therapy staff to facilitate problem solving and coordination of services; may serve as an educational resource.
  • Mentors and provides oversight to new caregivers, physical therapist assistants, and/or therapy aides; provides clinical instruction and training to students.
  • Recommends and participates in development of new services and programs, quality improvement and monitoring activities.
  • Participates in staff meetings, patient care meetings, educational in-services, caregiver orientation and other professional activities as assigned.
  • Performs other duties as assigned.
What you bring
  • Bachelor’s, Master’s or Doctorate Degree in Physical Therapy from an ACPE-accredited program (foreign equivalent accepted).
  • Required:

    Physical Therapist License, State of Oregon.
  • Required Upon Hire:
    Basic Life Support (BLS).
