This position reports to a Licensed Professional and Agency Director(s).
Education and Experience:Preventive professional means, within the MHIDD/SAS system of care:
- An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of South Carolina by the South Carolina Board of Nursing who also has four years of full-time accumulated experience in MHIDD/SA with the population served; or
- A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated MHIDD/SA experience with the population served, or a substance abuse professional who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
- A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated MHIDD/SA experience with the population served, or a substance abuse professional who has two years of full time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; or
- A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated MHIDD/SA experience with the population served, or a substance abuse professional who has four years of full time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
- Development, monitoring and revision of person-centered plans and plans of care
- Coordination of other services and supports related to maintaining an individual's health, safety, and well-being in the community
- Facilitating person centered planning, team meetings,
- Facilitating the service delivery process, including initial assessment, ongoing monitoring of care and services
- Monitoring to assure quality care and continued appropriateness of services
- Submitting person centered plans to Value Options or LME for authorization
- Ensuring linkage to the most clinically appropriate and effective services
- Supportive counseling to address the diagnostic and clinical needs of the recipient
- Assistance with therapeutic interventions
- Psychoeducation and training of family, unpaid caregivers,
- Relapse prevention and disease management
- Ongoing symptom monitoring and management
- Service coordination activities within the established person-centered plan
- Documenting service provision accurately and timely
- Attending staff meetings
- Quality improvement activities
- Participation in continuing education
- Perform NCI Training
- Marketing
- Assist the director
- Other duties as assigned by supervisor or director
