Associate Center Clinical Director - Cedar Hills, Jacksonville

Position Overview

We’re a family‑owned, physician‑led organization that is transforming primary care for seniors. We’re looking for an Associate Clinical Director to supervise and train PCPs at a center and to maximize clinical quality, efficiency, and satisfaction.

• Supervise and train primary care providers (PCPs) in assigned center.
• Allocate time to direct patient care as a PCP and other duties based on direct reports.
• Maximize core model execution, improving clinical quality, efficiency, outcomes and satisfaction.
• Provide care for patients with acute and chronic illnesses in the older adult population.
• Seek consultation from specialists when appropriate and maintain detailed patient care.
• Engage hospitalists, transitional care teams, case managers, and other resources.
• Lead care team (medical assistant, facilitator, coordinator) for office‑based patients; engage transitional care team for others.
• Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly and support center leadership as needed.
• Monitor daily center census and manage center operations, covering for other providers.
• Recruit patients and interview additional providers as required.
• Be available for walk‑in visits and prompt communication to build patient trust.
• Deliver patient‑centered, evidence‑based care and continually improve clinical and operational performance.
• Promote physician wellness and self‑care to sustain high quality care.

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
• Current, active MD licensure in the state of employment.
• Minimum of 1‑year clinical experience in geriatric, adult or family practice (preferred).
• Board certification in the terminal specialty (preferred); board‑eligible is required.
• Completion of Chen Med training, including Masterful Conversations and meeting facilitation.
• Basic Life Support (BLS) certification.
• Current DEA number for schedule II‑V controlled substances.
• Proficiency with Microsoft Office Suite and related applications.
• Willingness to travel locally as needed, nationally for initial training, and occasionally regionally and nationally for recruiting or training.
• Fluency in English; fluency in other languages may be required for some centers.
• Ability to exercise independent judgment.

Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other applications used in the company. Ability and willingness to travel locally as needed, nationally for initial training, and occasionally regionally and nationally for recruiting or training purposes. Fluency in English, verbal and written; other languages as required. The role requires use and exercise of independent judgment.

• Pay range: $238,832 – $341,189.
• Base hourly rate or base annual full‑time salary; final compensation depends on experience, education, geographic location and other factors.
