This position is based in Sarasota, FL, serving patients in South Sarasota (Englewood, Venice, North Port, Port Charlotte, Punta Gorda).

• Deliver high-quality nursing care to patients in their homes based on individualized care plans.
• Perform a variety of tasks, including:
  • Acute and chronic disease management.
  • Medication management and administration.
  • Assessing patient conditions and reporting changes.
  • Developing tailored, goal-oriented treatment plans.
  • Post-surgical rehabilitation.
  • Educating patients and caregivers to support recovery and independence.
  • Pain management and wound care.

• Unencumbered Florida LPN License (required).
• Valid driver’s license, auto insurance, and reliable transportation (required).
• IV Certification (required).
• Experience in acute care or hospital settings (preferred).
• Strong assessment and care planning skills.
