Join Center Well Home Health as a Medical Social Worker in a home health setting. You will assess patient emotional and social needs, develop individualized care plans, and coordinate community resources.

$59,300 - $80,900 per year (full time, 40 hrs/week). Pay reflects skills, experience, and geographic location.

• Assess patients’ social and emotional state and adjust care plans accordingly.
• Identify financial and community resources and facilitate access.
• Educate patients and families on social and emotional responses, and provide ongoing assessment.
• Assist physicians and the interdisciplinary team in understanding social and emotional factors related to health.
• Participate in periodic re-evaluation of the physician’s plan of care.
• Observe, record, and report changes in patients’ conditions to the Clinical Manager.
• Participate in discharge planning and care coordination activities.
• Supervise, instruct, and evaluate the performance of the Social Work Assistant (BSW).
• Maintain and submit required documentation (case conferences, visit reports, progress notes).

• Masters or doctoral degree from an accredited school of social work.
• Licensed social worker in the state of practice; state licensure required.
• Valid driver’s license, reliable transportation, and auto insurance.
• Current CPR certification.
• Minimum one year of experience as a social worker in a health care setting, home health, or hospice.
• Knowledge of discharge planning and community resource referral.
• Excellent oral and written communication and interpersonal skills.
