Your Specific Responsibilities

This position, an employee of the Nassau County Health Department (NCHD), works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. Employee will be operating a state vehicle to service clients within the Nassau County area.

This position reports directly to the Executive Community Health Nursing Director-SES.

  • Perform complete physical examination, compile, and evaluate medical history and physical findings.
  • Manage care of clients with chronic or acute conditions.
  • Select and use diagnostic tests and procedures in the diagnostic process.
  • Identify and evaluate physical and behavioral problems, diagnose, and implement treatment plans.
  • Monitor therapies and medications and alter treatments according to variations in patient response.
  • Coordinate the implementation and continuity of care.
  • Share timeframe for follow-up appointments.
  • Counsels and educates client and/or family regarding healthy living and healthiest weight.
  • Discusses the risks of tobacco product use, second‑hand smoke, and vaping. Provides resources to assist with tobacco and/or vaping cessation.
  • Makes referral to internal and external providers for care outside the APRN scope and/or specialty care.
  • Orders HIV and STD Screening for all sexually active at‑risk clients a minimum of yearly.
  • Provides pre/post‑test counselling for HIV screening.
  • Evaluates and treats all ages of clients presenting with symptoms of a STD or requesting STD testing.
  • Provides all Family Planning services within the APRN scope and Title X requirements.
  • Reviews ordered labs and other test results promptly (ideally within one business day and no more than two business days).
  • Notifies clients or delegates notification of clients of positive lab and other test results.
  • Real time documentation of all care and client communication in HMS.
  • Orders medications and documents services before seeing the next client.
  • Provide input to policy development.
  • Perform other related duties as required within the scope of APRN license and/or credentials/certifications.
  • May need to use personal or county vehicle for work related duties.
Immunizations
  • Encourages ACIP/CDC recommended immunizations for age and/or lifestyle.
  • Maintains a contemporary knowledge of immunization changes and updates.
Other Duties
  • Performs lab services needed by clients (microscopy).
  • Participates in the evaluation and selection of flyers and brochures available for our clients.
  • Assist in the orientation of new employees.
  • Precept students as requested by leadership.
  • Serve as relief nurse in clinics when a nursing shortage exists.
  • Performs all the other duties as required.
