Job Description
LPN Resident Care Coordinator Position
This is a supervisory position in assisted living. Experience required. The position is full time, 2p-10p shift, with weekend availability. Up to $30.00/hour plus shift differentials based on experience.
Responsibilities:
- Assist with medications when needed.
- Assess residents and work with physicians to ensure best healthcare for residents.
- Supervise staff and ensure excellent customer service.
- Assist with admissions and discharges.
- Respond to residents’ needs in a timely manner with memory care residents and families.
- Conduct assessment, care planning, audits, family council involvement, and related tasks.
