Responsibilities
  • Participate in facility committees responsible for establishing and implementing medication-related policies and procedures within the ICU.
  • Primarily responsible for the direction of the critical care service line within the hospital.
  • Identify how drug costs may be minimized through appropriate use of drugs in the ICU and through implementation of cost-containment measures.
  • Participate in nursing, physician, or ancillary team onboarding and staff meetings with the ICU as appropriate to explain clinical pharmacy services and educate on new drugs or updated clinical guidelines.
  • Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
  • Accurately compound and dispense pharmaceuticals.
  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
  • Sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection.
  • Provide accurate and timely medication information to healthcare staff and patients.
  • Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
  • Document all clinical activities and interventions accurately and completely.
  • Complete notification reports regarding medication errors and adverse drug reactions.
  • Effectively supervise pharmacy operation activities and provide support to the whole pharmacy team.
Qualifications
  • B.S. or Pharm

    D from and ACPE accredited School of Pharmacy required
  • Minimum of two years of hospital experience is required or Resident trained preferred
  • Active pharmacist license in good standing with the appropriate State Board of Pharmacy
