Optum Home & Community Care is seeking a House Calls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. Flexible scheduling is offered (20–40 hours). No on-call, weekends, or holidays, with a supportive, interdisciplinary team environment.
You will assess health status, coordinate with primary care, educate patients, and address social determinants of health, delivering quality care in patients' homes.
