Center Well Home Health in Florida is seeking a Medical Social Worker to evaluate and develop a plan of care for home health patients, addressing emotional and social needs and supervising the Social Worker Assistant when involved.
You will collaborate with the interdisciplinary care team, assess social/emotional needs, and connect patients with community resources to support recovery.
Florida licensure, CPR, a valid driver’s license, and the ability to travel locally are required.
