Uplift Home Care, LLC is seeking a dedicated Occupational Therapist (OT) to join our growing home health team serving Odessa, Florida!

Help Seniors and Veterans improve daily functioning, regain independence, and remain safely in their homes while working with a team committed to compassionate, individualized care.

  Begin once onboarding and credentialing requirements are complete.

• Evaluate patients in their homes and assess functional abilities and individual needs.
• Develop individualized treatment plans and measurable goals.
• Provide therapeutic interventions to improve activities of daily living, safety, and independence.
• Educate patients and caregivers on home exercises, adaptive techniques, and fall prevention.
• Monitor patient progress and adjust treatment plans as clinically appropriate.
• Complete accurate, timely, and compliant clinical documentation.
• Collaborate with the interdisciplinary care team to support positive patient outcomes.

• Graduate of an accredited Occupational Therapy program.
• Active Florida Occupational Therapist license.
• Current CPR certification.
• Required AHCA Level II background screening.
• Home health experience preferred.
• Strong clinical judgment, communication, and organizational skills.
• Ability to work independently and manage field visits effectively.
• Valid driver's license, current auto insurance, and reliable transportation.
