Location: Tequesta

The Case Manager supports mothers residing in the maternity home by connecting them to community resources, coordinating transportation, and assisting with individualized service plans. This role also ensures accountability, structure, and consistency in daily routines by supporting residents with task management, room check-ins, and occasional program facilitation. The Case Manager works closely with the Resident Program Director and plays a vital role in fostering an environment of growth, stability, and encouragement.

Duties and Responsibilities
  • Provide case management services for all residents, ensuring that each mother is guided toward appropriate resources, including healthcare, education, employment, housing, and social services.
  • Assist residents in understanding and accessing community resources, empowering them to advocate for themselves.
  • Coordinate and provide transportation for residents to appointments, job interviews, childcare, educational programs, or other essential activities.
  • Develop, implement, and monitor individualized service plans in collaboration with the Resident Program Director.
  • Create and complete daily/weekly to-do lists for residents to help them stay organized and on track with their goals.
  • Conduct daily room check-ins to ensure cleanliness, safety, and accountability in the living spaces.
  • Lead or assist with devotions when directed by the Resident Program Director to help maintain consistent spiritual activities in the home.
  • Maintain accurate documentation of case management activities, progress notes, service referrals, and follow-up actions.
  • Attend weekly meetings with the Resident Program Director to review resident progress and update individualized plans.
  • Participate in required trainings (e.g., mental health, trauma‑informed care, parenting, or other areas) as directed by the Resident Program Director.
  • Foster a supportive, respectful, and safe environment for residents while modeling positive problem‑solving and decision‑making skills.
  • Collaborate with staff and volunteers to ensure residents receive consistent support and programming.
  • Meet weekly with residents individually to track their progress, identify additional needs, and resolve any challenges.
  • Maintain complete resident files.
  • Occasionally transport residents to various appointments and activities as needed.
  • Participate in community meetings.
  • Participate in weekly staff meetings, direct care team.
  • Participate in staff development opportunities and identify training as needed.
  • Occasionally handle additional tasks as needed.
Qualifications and Experience
  • Bachelor’s degree with minimum two years’ experience in social services or related work, preferably in nonprofit context.
  • Ability to flourish in a collaborative, team‑oriented environment.
  • Ability to multi‑task, organize, and meet deadlines.
  • Excellent verbal and written communication skills.
  • Proficiency with Microsoft Word and Excel.
  • Ability to aim for long‑term goals while executing the necessary short‑term achievements.
  • Commitment to creating and sustaining an inclusive work and living environment and to embracing diversity as a core value of the organization.
  • Qualifying Level 2 background check.
Work Environment
  • Workplace is a residential group home for vulnerable, single pregnant women aged 18‑30.
  • Workplace culture emphasizes healthy, trustworthy relationships, clear communication, conflict resolution skills, responsibility, and balanced responses to stress.
  • Case Manager collaborates with the direct care team to manage the residents’ progress toward program goals.
  • Light travel for mostly local appointments, activities and events.
  • Occasional lifting, carrying, pushing and/or pulling objects weighing up to 20 pounds.
