Job Description
Position OverviewA 280+ bed acute care hospital is seeking an experienced RN Case Manager (Evenings) to join the Case Management team. In this role, you will coordinate interdisciplinary care, conduct utilization review, determine appropriate level of care, and support safe, timely discharges across ED, Med/Surg, Tele, Neuro, ICU, and PCU populations.
You will report directly to the Director of Case Management
.
- Complete comprehensive clinical and psychosocial assessments and develop criteria-based plans of care with clear discharge goals.
- Lead interdisciplinary huddles and collaborate closely with physicians, nursing, therapy, pharmacy, and social work.
- Conduct medical necessity reviews at admission and throughout hospitalization; elevate level‑of‑care issues as needed.
- Manage observation status appropriateness and convert to inpatient status when indicated.
- Perform concurrent/utilization reviews; document and communicate with third‑party payers.
- Identify barriers to throughput; resolve issues to reduce LOS and prevent avoidable days and readmissions.
- Coordinate post‑acute care services and connect patients/families with community resources.
- Document case management interventions and track/trend variances; contribute to performance improvement initiatives.
- Support compliance with regulatory standards and assist with staff education and protocol implementation.
Required
- Active Florida RN license or Compact RN (must obtain FL license within 90 days if Compact).
- 3 years recent acute RN Case Management experience
, OR - 3 years acute care RN experience (Med/Surg, Tele, Neuro, ICU, PCU, or ED) with strong UR/throughput skills.
- Associate Degree or Diploma in Nursing.
Also Considered (if acute background is met)
- Home Health or Insurance Case Management experience.
- APRN license (if current and compliant).
Preferred
- BSN
- Case Management/UR certifications (CCM, ACM, RN-BC)
- Base Pay: $31.94–$43.92/hr
- Differentials:
- +$2.50/hr Evenings
- +$2.00/hr Weekends (up to +$4.50/hr stacked)
- Sign-On Bonus: $10,000 total
- $2,000 on 1st & 2nd paycheck
- $2,000 at 6, 12, 18, and 24 months
- Relocation Assistance: Case-by-case
