Job Requirements
  • Active Florida Nurse Practitioner License (unencumbered)
  • Master's Degree in Nursing from an accredited program
  • Strong clinical background in Infectious Disease
  • Geriatric or adult medicine experience preferred (skilled nursing, long‑term care, hospice, palliative care, or similar settings)
  • Effective communication with elderly and chronically ill patients and families
  • Ability to meet a minimum of 12 patient visits per day (bonuses for exceeding target)
  • Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus
Key Responsibilities
  • Perform comprehensive physical examinations and evaluate changes in patient conditions
  • Prescribe medications and treatments as needed
  • Develop patient care policies and facilitate referrals to specialists
  • Coordinate advanced care planning and discharge visits
  • Ensure timely communication and documentation of patient assessments, treatment plans, and follow‑ups
  • Support the physician‑patient relationship and address gaps in care
Salary & Compensation
  • Annual Pay: $105,000 – $130,000 (based on experience)
  • Monthly Bonus Opportunities:
    Performance‑based bonuses
