Licensed Practical Nurse

The Palms Home Care is hiring two full-time LPNs to provide skilled, compassionate care to patients in their homes.

• West/Central Tampa: Serving patients from West Tampa through Central Tampa.

• Brandon area: Serving patients within an assigned territory extending into Brandon.

• Full-time salaried position

• $500 monthly car allowance

• Clinical leadership and team support

• Provide skilled nursing care under the supervision of an RN and according to the established plan of care.

• Perform wound care, administer ordered medications and treatments, and monitor patient response.

• Educate patients and caregivers on medications, disease management, and home care instructions.

• Promptly communicate changes in patient condition to the supervising RN.

• Complete accurate, timely documentation.

• Coordinate with the care team to support continuity of care.

• Participate in one on-call rotation per month.

• Active LPN license authorizing practice in Florida

• Previous home health experience required

• Current CPR certification

• Valid driver's license, reliable transportation, and automobile insurance

• Strong communication, organization, and time management skills

• Well Sky/Kinnser experience preferred
