Max Health is seeking an Onsite Inpatient Case Manager to support hospitals in Sarasota/Lakewood Ranch, coordinating care transitions after ED visits, hospitalizations, or SNF stays. You will collaborate with clinical teams, providers, and families to ensure safe, timely discharges and improved patient outcomes.
Typical hours are Monday–Friday 8:00am–5:00pm with some flexibility. A $500 signing bonus is offered after 30 days.
