The Intake and Family Coach Coordinator is the key role taking end-to-end operational responsibility for referrals to hosting or family friend arrangements, and oversight of all the Safe Family engagements via volunteer family coaches.
• Provide clinical/safety oversight of SFFC program implementation.
• Provide supervision and training for SFFC volunteer family coaches serving in each of the partner churches.
• Develop relationships with referral sources and ensure # of referrals meets performance metrics
Provide emergency, on-call support to staff and volunteers in the SFFC program.
Oversees and manages the process from referral to placing in a host family home or a family friend assignment. This involves overseeing initial calls, assessment, identification of an appropriate hosting arrangement, coordinating transportation and pickup, assigning family coach and family coach supervisor, and ensuring the host families are prepared to care for the child.
Responsibilities
Handling incoming referrals. Being the point person for incoming phone calls and emails from agencies working with at-risk families, such as social workers; and also dealing directly with in-crisis parents. Creating a case file on the database and entering all relevant info.Information gathering and screening. Following up with referrals and screening them according to predetermined risk thresholds, mainly by telephone. Being friendly but discerning, obtaining relevant facts and information, and building up a picture of the crisis. Screening out inappropriate placements at an early stage. Communicating with other agencies who may hold relevant information pertinent to the case.Ensure that all referral calls gather appropriate information, are assessed thoroughly, and appropriate decisions are made whether to accept the placement/arrangement or not.Screen out youth who: are a danger to themselves or others, whose problematic behavior cannot be managed in a family-like setting, etc.Provide an initial assessment of the parent and identify targeted behaviors that need to be resolved and approximate the length of stay in a host family home.Make a determination that the hosting of the child will not encourage the parent to continue in problematic behavior.Make every effort to arrange placements within the same day of the request if that is desired by the parent or referral source. Will respond within an hour for urgent placements.Make sure that placement information is appropriately and accurately entered into the database.Ensure the host family has all the necessary information and paperwork to care for the child effectively.Create innovative strategies for identifying available families and facilitating the child to the host home.Any other duties as may be assigned.Responsibilities
Responsible for completing home studies and clinical interviews of all host families and family coaches, approve/denying volunteers, and completing paperwork from home studies.Responsible for supervising, training, and supporting Family Coach and Host Family volunteers on all aspects of the hosting process, including being available on an emergency, on-call basis.Ensures family coaches help parents set goals and efficiently work to fulfill those goals.Assist with maintaining files for all Safe Families volunteers ensuring that they are complete with regard to all the necessary particulars.Consult with family coach supervisor on referrals, determine program “fit” and match to host family, and assist with establishing hosting arrangements.Responsible for quality of client care: to plan, develop, and administer programs such that the basic, developmental, and therapeutic needs of clients in care are metParticipate in tracking/reporting of information pertinent to recruitment, training, and clearing/certifying volunteers.Assist in the Safe Family selection, transportation, and placing of children into host family homes and other supportive services with Safe Family volunteers.Assist in the connection of Safe Families volunteers, local churches, and community members.Maintain current knowledge pertaining to Safe Families family coaching; continue…
