We’re hiring a full‑time Director of Social Services to oversee our Social Work Department for long‑term and acute care patients. Help residents thrive, guide families, and lead a team dedicated to exceptional care.
What You’ll Do:- Lead social services for residents and families.
- Develop social histories, assessments, and care plans.
- Participate in interdisciplinary care meetings.
- Support discharge planning and placement.
- Ensure compliance with state and federal regulations.
- Bachelor’s in Social Work or related field (Master’s preferred).
- Strong interpersonal & leadership skills.
- Experience in long‑term care or healthcare social services.
- Knowledge of state and federal social services guidelines.
