Licensed Practical Nurse role at Sevita (REM Community Services, part of the Sevita family).

Responsibilities
  • Monitor health conditions of individuals served and ensure quality services are delivered.
  • Administer medications, document care, and oversee staff who administer medications.
  • Provide prescribed medical treatment per physicians' orders and personal care services.
  • Conduct assessments and implement health‑care services in each Individual Service Plan (ISP), responding promptly to changes.
  • Train staff on universal precautions, infection control, and medication administration.
  • Act as liaison and advocate with other medical and professional service providers.
  • Educate individuals served on self‑medication, prescribed treatments, sex education/STD prevention, and smoking cessation.
Qualifications
  • Associate’s Degree in Nursing or related field, or graduate of an accredited LPN program.
  • One year of experience working with the population served in a nursing capacity.
  • Current state LPN or LVN license.
  • Current driver’s license and auto insurance.
  • Certifications required by state licensing regulations such as CPR/FA, HIV/AIDS certification, BBP, and ACT training.
  • Effective communication skills with staff, individuals served, and families.
  • Commitment to improving the quality of life for individuals served.
