Social Worker/Care Coordinator ED Queens Nights

New York-Presbyterian/Queens, located in the wonderfully diverse neighborhood of Flushing, Queens, is a community teaching hospital affiliated with Weill Cornell Medicine, serving Queens and metro New York residents.

We are seeking a Social Worker for our Emergency Department. Experience a place where Social Workers are respected. Utilize your inpatient experience to provide a variety of psychosocial services, coordinate discharge planning and continuum of care for patients and families from across the world. This role primarily consists of direct management and coordination of a patient's care plan across different providers.

This is a night shift 7PM - 8:15AM;
Monday – Friday with rotating weekends​ at NYP/Queens Hospital in Flushing, NY.

Preferred Criteria

• At least one year of inpatient Medical/Surgical experience

• Discharge Planning experience

• LCSW

Required Criteria

• Master’s Degree in Social Work (MSW)

• New York State License as a Licensed Masters Social Worker (LMSW)

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
