As a Home Health Physical Therapist at Center Well, you will provide skilled, patient‑centered care in the comfort of clients’ homes. You will design and implement individualized rehabilitation programs to help patients regain strength, mobility, and independence, and will collaborate closely with physicians and the care team.

Pay per visit: $54.00 – $76.00
Base pay: $85,400 – $117,500 per year

• Plan and administer prescribed skilled physical therapy treatment and training for patients with injuries, illnesses, and functional disabilities.
• Screen the patient’s physical strengths to assist the physician in evaluating function and record findings to develop or pursue treatment programs.
• Develop and implement a conditioning/rehabilitation program consistent with the physician’s Plan of Treatment and the patient’s goals.
• Consult with physicians and clinical team members to obtain additional patient information and revise therapy treatment plans.
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions and care plans that reflect the current plan of care for therapy; supervise and evaluate staff performance.
• Accurately, promptly, and thoroughly document patient care observations, interventions, and evaluations.
• Report patient progress to the physician, Clinical Manager, staff, patient, and family; submit evaluation, treatment plans, progress reports, and discharge summaries.

• Degree from an accredited Physical Therapy Program (APTA approved).
• Minimum of one year of physical therapy experience preferred.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation required.
