Responsibilities

Perform comprehensive assessments of high-risk and hospice patients

Collaborate with primary care providers and interdisciplinary teams to develop and implement individualized care plans

Continuously reassess patient needs to ensure appropriate, cost-effective levels of care

Arrange non-medical supports (e.g., housing, transportation) to support treatment compliance

Coordinate care transitions across settings as needed

Communicate and collaborate with physicians, social workers, discharge planners, and claims professionals

Engage specialty resources to support optimal patient outcomes

Maintain detailed documentation of clinical, functional, and fiscal outcomes

Required Qualifications

Must have RN License for Wisconsin

Wound Care experience

Certified CPR/BLS

Valid driver's license, reliable vehicle, and valid auto insurance

Home Care Home Base (HCHB) highly desired;
Matrix Care or other EMR acceptable

IDG documentation, recertification, and pain management experience

Associate's or Bachelor's degree in Nursing or related field

Case Management certification
