SSM Health is seeking a compassionate Registered Nurse to join our Hospice team providing home care across Wisconsin. You will travel to patient homes, CBRS, assisted living facilities, and hospitals to assess needs and coordinate a care plan under the attending physician.
Responsibilities include discharge planning, documentation, wound and IV care, vital signs, and collaboration with a multidisciplinary team to support end-of-life care and patient comfort. Four days/week with on-call weekends.
