Registered Nurse – East Central Planning and Development District

East Central Planning and Development District (ECPDD) is seeking a licensed nurse to work in the Elderly and Disabled Waiver Program. The role involves coordinating the Case Management Program, planning and arranging home and community-based services, and working with a Licensed Social Worker case manager to screen and determine eligibility for clients. The position offers excellent fringe benefits and requires no nights, weekends, or on-call responsibilities.
Responsibilities- Coordinate the Case Management Program, including planning, arranging, and coordinating home and community-based services for clients in a timely and cost-effective manner.
- Maintain regular contact with service providers and clients.
- Collaborate with a Licensed Social Worker case manager to screen referred clients and assess eligibility.
- Bachelor’s degree (minimum).
- Registered Nurse license.
- Valid driver’s license.
