• Plan and administer skilled physical therapy treatment for patients with injuries, illnesses, and functional disabilities to achieve the highest level of physical function.
• Screen and test patients’ physical strength to assist the physician in evaluating function and develop treatment programs with measurable objectives.
• Develop and implement a conditioning/rehabilitation program consistent with the physician’s Plan of Treatment and overall patient goals.
• Collaborate with the physician and clinical team to revise the therapy treatment program and Plan of Treatment as needed.
• Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aide staff, supervise staff performance, and monitor documentation accuracy.
• Accurately document patient care observations, interventions, evaluations, and physician orders in a timely manner.
• Report patient progress to the physician, Clinical Manager, staff, patient, and family; submit treatment plans, progress reports, and discharge summaries.

• Doctorate from an accredited Physical Therapy Program approved by the APTA.
• Minimum one year of physical therapy experience preferred.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.
