Fort Health Care is a patient-centered hospital and healthcare system serving individuals, businesses and families in Jefferson, Dane, and Walworth Counties and we are seeking a School-Based Behavioral Health Therapist, licensed or in-training, to join our Behavioral Health team. This position will support the Whitewater School District K-12.
Shift:
80 hours per bi-weekly pay period

8:00 am-4:00pm; no weekends/no holidays
The School-Based Behavioral Health Therapist provides individual and/or family therapy to children and adolescents within a school-based setting. The School-Based Behavioral Health Therapist is responsible for completing psychosocial assessments, treatment planning, evidence-based interventions, and required chart documentation for assigned clients. The School-Based Behavioral Health Therapist collaborates with other providers and clinical staff within FHC, school personnel, primary care and/or community supports as needed.
Responsibilities of this position include:
- Conducts comprehensive psychosocial assessments, formulates diagnosis, and monitors individual's treatment plan/interventions and updates treatment plan(s) as required.
- Uses evidence-based therapeutic treatment modalities for individual, family therapy or group therapy.
- Provides crisis intervention/safety planning as needed.
- Prepares timely clinical documentation in electronic medical record in accordance with policies and regulatory standards.
- Actively participates in multi-disciplinary clinical collaboration and consultation within Fort Health Care Behavioral Health.
- Works collaboratively with other professionals within the school system/district regarding school-based referrals.
- Maintains caseload as assigned; case management and/or aftercare planning as needed.
- Demonstrates ability to work with diverse client population within the community for those with mental health and/or substance use disorders.
- Provides community education, marketing outreach, and performance improvement activities as requested.
Requirements of this position include:
- Masters Degree in Counseling;
Social Work or Marriage and Family Therapy. - Ability to work independently and collaboratively. Experience working with diverse populations. Excellent interpersonal, communication, and writing skills. Experience with computers and demonstrated proficiency with software programs.
- Experience with documentation in electronic medical record, understanding of medical terminology preferred.
- Current WI License: LCSW, LPC, LMFT, APSW, APSW; LPC-IT
