Location: Fort Atkinson, Wisconsin

• Graduate of an accredited nursing program (Associate, Bachelor’s, or Master’s).
• Current Registered Nurse (RN) license in the state of practice.
• 2+ years of hospice or home‑health clinical experience.
• 2+ years of supervisory or management experience.
• Oversee coordination of patient care and assign appropriate staff to each case.
• Support clinicians with real‑time guidance, mentoring, and clinical problem‑solving.
• Conduct case reviews, home visits, and clinical audits to ensure high‑quality care.
• Manage hiring, training, supervision, and evaluation of clinical staff.
• Assist in ongoing staff education, orientation, and performance improvement efforts.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.

• Schedule:
  
  Monday through Friday, full‑time position with on‑call responsibilities.
