• Provide compassionate personal care to clients according to their care plans, including assistance with bathing, grooming, and mobility.
• Perform household tasks such as meal preparation, light cleaning, and laundry.
• Offer companionship and emotional support to clients.
• Escort clients to appointments, shopping, and other activities as needed.
• Communicate changes in client condition promptly and accurately to supervisors.
• Maintain thorough documentation of care provided and client status.

Care Plan Adherence - Care is delivered according to established care plans.

Documentation Accuracy - Notes submitted on time and without errors.

Schedule Reliability - 95%+ on‑time arrival for shifts.

By signing this agreement, the employee confirms that they are legally authorized to work in the United States and will provide documentation to verify their work authorization status as required by federal law (Form I-9).
