Occupational Therapist
Storybrook Care & Rehabilitation, a family‑owned long‑term care facility in Northern Colorado, is seeking an Occupational Therapist to join our in‑house therapy team.
Pay range: $43.00 ‑ $47.00 per hour. Sign‑on bonus available.

Responsibilities- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care per regulatory and clinical standards.
- Utilize standardized assessments and evidence‑based practice to support clinical interventions.
- Provide rehabilitative, skilled and medically necessary treatment intervention to patients per regulatory and clinical requirements.
- Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
- Assess patient progress and adjust treatments accordingly in the clinical record.
- Evaluate treatment outcomes for effectiveness and modify services as needed.
- Consult and counsel patients, families, caregivers, and other service providers regarding physical disorders.
- Communicate with supervisors and other health team members about patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summaries per regulatory requirements.
- Document required information in the electronic documentation system and maintain patient records per company policy.
- Accurately record treatment minutes for services provided as required.
- Participate in required meetings: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
- Attend facility and company training, in‑services, and conferences.
- Supervise and support physical therapist assistants, rehabilitation aides, and/or students.
- Adhere to NBCOT/AOTA and state practice acts, code of conduct, and ethics during professional practice.
- Adhere to company policies, procedures, and code of conduct.
- Hold an active state license.
