Occupational Therapist

Storybrook Care & Rehabilitation, a family‑owned long‑term care facility in Northern Colorado, is seeking an Occupational Therapist to join our in‑house therapy team.

Pay range: $43.00 ‑ $47.00 per hour. Sign‑on bonus available.

Responsibilities
  • Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care per regulatory and clinical standards.
  • Utilize standardized assessments and evidence‑based practice to support clinical interventions.
  • Provide rehabilitative, skilled and medically necessary treatment intervention to patients per regulatory and clinical requirements.
  • Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
  • Assess patient progress and adjust treatments accordingly in the clinical record.
  • Evaluate treatment outcomes for effectiveness and modify services as needed.
  • Consult and counsel patients, families, caregivers, and other service providers regarding physical disorders.
  • Communicate with supervisors and other health team members about patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summaries per regulatory requirements.
  • Document required information in the electronic documentation system and maintain patient records per company policy.
  • Accurately record treatment minutes for services provided as required.
  • Participate in required meetings: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Attend facility and company training, in‑services, and conferences.
  • Supervise and support physical therapist assistants, rehabilitation aides, and/or students.
  • Adhere to NBCOT/AOTA and state practice acts, code of conduct, and ethics during professional practice.
  • Adhere to company policies, procedures, and code of conduct.
  • Hold an active state license.
