Physical Therapy Assistant – Story brook Care & Rehabilitation

Base Pay Range: $32.00/hr – $39.00/hr

We are a family‑owned, long‑term care facility located in Northern Colorado, serving a diverse group of residents with low staffing ratios and a highly experienced clinical team.

Join our in‑house, close‑knit therapy team that collaborates closely with our interdisciplinary team and values each member as appreciated, seen, and heard.

• Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist.
• Communicate with the Physical Therapist any pertinent information concerning patient progress, concerns, and plans.
• Be proficient in therapeutic techniques to treat patients based on their presentation and instructions given by the Physical Therapist.
• Participate in patient plan‑of‑care meetings if requested by the Director of Rehabilitation.
• Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedures.
