Lutheran Family Services Rocky Mountains (LFSRM) is a nonprofit, human services agency committed to community safety, support, and resiliency. With a history dating to 1948, LFSRM provides support, guidance, and resource coordination to individuals and families regardless of race, religion, gender identity, sexual orientation, or age through family and refugee services.
We’re looking for an experienced UC Case Lead to manage Unaccompanied Children (UC) in federal custody as they transition to long‑term foster care. The role involves casework, supportive services, coordination with community agencies, and ensuring legal and health requirements are met.
Job Summary & Responsibilities- Provide casework and supportive services to children in foster care, facilitating integration into the local community and supporting UC access to education, legal, medical and dental services.
- Assist family members identified for potential sponsorship and help with application processes.
- Communicate with stakeholders effectively to ensure case continuity.
- Maintain documentation, assessments, service plans, and progress notes; participate in court hearings, placement review meetings, and school meetings.
- Provide support, guidance, and training to foster parents and ensure ongoing assessment of needs of youth and families on caseload.
- May participate in on‑call rotation and transport clients as needed.
- Familiarity with social work practice, child welfare system, and relevant regulations.
- Case‑management skills: assessment, care planning, coordination, and advocacy.
- Crisis‑intervention experience with mediation best practices.
- Understanding of legal requirements in child welfare.
- Sensitivity to diverse cultural communities.
- Professional communication with interdisciplinary teams.

- Bachelor’s degree in Social Work or related behavioral science;
Master’s preferred. - Minimum two years of child welfare/case management experience; experience with refugees/immigrants is a plus.
- Strong bilingual English and Spanish preferred.
- Valid driver’s license with personal auto liability insurance.
- Must present proof of vaccination for Varicella, MMR, Tdap, and annual Influenza within 120 days of hire.
Salary: $23.03 – $24.22 per hour. Sign‑on bonus: $3,000.
