ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES
- Participates in dietitians staff meetings and collaborates with updating and developing department policies and procedures.
- Participates, as requested, in committees relating to nutrition, food service, bioethics, performance improvement/quality assurance and department related issues.
- Prepares reports and materials to meet program and CMS requirements; including data records, notes, meeting minutes and other information.
- Monitors inventory of program equipment, supplies, patient supplements and materials. Ensures adequate availability of educational materials to conduct work activities.
- Performs general/clerical tasks, which may include answering telephone calls, scheduling appointments, making copies, sending/receiving faxes, filing documentation, processing incoming/outgoing mail, or performing light housekeeping tasks.
- Re-assesses patient’s nutritional status to determine appropriate nutrition intervention(s) and recommendations. Provides recommendations to physicians and healthcare providers regarding lab tests and need of referrals.
- Collaborates with providers to identify patients in need of nutrition referrals or counseling; accept or transmit verbal or electronically transmitted orders from physicians consistent with an established protocol to implement medical nutrition therapy.
- Implements the Nutrition Care Process to develop an individualized plan that is consistent with the patient’s cultural, social, and physiological needs, and communicates the treatment plan to patients, family members and caregivers.
- Coordinates services with other departments or outside facilities and provides information on community resources, and programs to maximize patient’s nutritional well-being.
- Serves as a resource to other healthcare providers and works with the interdisciplinary team to establish, document and update patient’s plan of care to achieve goals.
- Selects and develops educational materials and methods appropriate for individual patient and groups.
- Promotes compliance with regulatory agencies and accreditation requirements by following the institutional and departmental policies and procedures.
- Develops activities and educational programs for patients with diabetes and collaborates with the Special Diabetes Program (SDPI) to achieve goals.
- Stays abreast on diabetes care and education by maintaining a minimum of 15 CEU’s per year and submits evidence of completion to the Clinical Dietitian Manager and the Pharmacy ADCES Credentialing personnel.
- Supports the inpatient setting by providing coverage in the absence of the inpatient dietitian and complete consults in the Electronic Health Record (EHR) according to department policies.
- Supports the inpatient Dietitian and the Nutritive Services Department by reviewing and approving patient menus and nutritional supplements on a yearly basis.
- Identifies ways to improve patients care and provides recommendations to Clinical Dietitian Manager, the Director of Nutritive Services, and related committees.
- Documents nutrition interventions in EHR in accordance with policies, procedures and regulations.
- Maintains professional competencies and proficiency through continuing education. Attends staff meetings and outside workshops.
- Participates in routine record maintenance in compliance with FDIHB, Federal and related regulatory policies.
- Reviews and develops appropriate educational materials for professional and paraprofessional staff who provide services to patients and the community.
- Collaborates with other departments in the development of nutrition group classes and programs in the outpatient and in-patient settings.
- Performs other duties as assigned.
MINIMUM MANDATORY QUALIFICATIONS
Experience: One (1) year of direct work experience or one (1) year of supervised practice as a dietetic intern from an accredited didactic program in dietetics.
