Job TitleNurse Practitioner PACE
Job SummaryUnder the general supervision of the medical director of FLORIDA PACE CENTERS, provides direct primary medical care treatment, and counsel to all participants enrolled into the program.
Essential Job Functions- Performs comprehensive history and physical on new referrals; provides interval history and physical on established participants quarterly or as indicated.
- Manages patients with acute, episodic, and chronic illnesses.
- Assumes ongoing responsibility for each patient under her/his care.
- Refers participants to medical director or specialists as indicated.
- Admits participants to hospital, provides primary care with the Medical Director, updates multidisciplinary team regularly, utilizes consultants appropriately, and coordinates discharge planning.
- Manages care of participants in nursing homes through regular visits, telephone contact with staff, and admissions to hospital when necessary.
- Participates actively in family conferences, interdisciplinary team meetings, and contributes to the team.
- Participates in committees and work groups as assigned.
- Documents treatment in patient charts per established department guidelines.
Graduate from an accredited school of nursing.
Job Requirements- Advanced training and certification as an Advanced Registered Nurse Practitioner (ARNP) or advanced training and licensure as a Physician Assistant (PA).
- 2-3 years’ experience preferred; minimum of 1 year specifically working with frail elderly populations.
- Current ARNP/PA license by the Florida State Board of Nursing.
- BLS certification required.
- ACLS certification preferred.
- DEA License required.
- Knowledge of nursing care practices for rehabilitation, geriatric, and dementia populations.
- Demonstrates a caring attitude toward residents, patients, family members, employees, and other facility guests.
- Normal working conditions with adequate lighting and ventilation.
- Exposure to toxic or caustic chemicals; biohazardous waste; flammable, explosive gases.
- Ability to assist in transferring patients and ambulation, involving lifting, pushing, and pulling patients from 50-150 pounds.
- Routine exposure to blood/body fluids/tissues.
- Ensures resident / patient rights are adhered to.
- Demonstrates professionalism, accountability, and caring attitude consistent with Miami Jewish Empathicare®.
- Demonstrates excellent communication skills.
- Ensures confidentiality and security of patients’ medical information.
- Utilizes appropriate channels of communication.
- Speaks, reads, and writes English.
- Maintains calm under unusual occurrences.
- Adheres to organization’s mission, vision, and values.
- Participates in departmental activities, meetings, and in‑services and follows established guidelines.
- Maintains a safe working environment.
