Job DescriptionMatrix Home Care is currently seeking an experienced and compassionate LPN for per-diem skilled home health visits throughout Broward County.
The LPN will provide patient-centered care in the home setting. This is a great opportunity for an LPN who enjoys flexible scheduling and providing one-on-one care to patients in their homes.
Responsibilities:- Provide skilled nursing visits in the patient's home
- Perform wound care treatments as ordered
- Provide medication management and patient education
- Perform IV therapy as ordered and within scope of practice
- Complete lab draws as needed
- Monitor patient condition and report changes to the clinical team
- Follow the patient's plan of care
- Maintain accurate and timely clinical documentation
- Communicate professionally with patients, families, and care team members
- Maintain a safe and supportive care environment
- Prior home health experience preferred
- Experience with wound care, medication management, IV therapy, and lab draws preferred
- Strong clinical assessment and documentation skills
- Ability to work independently in the home care setting
- Professional, dependable, and compassionate
Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
