- Provide direct services to program participants, including participating as a member of the wraparound team, delivering youth development activities, and supporting assessments, service planning, linking and coordinating community resources, and all program components (e.g., field trips, community projects, employment activities, health education).
- Conduct Independent Living Life Skills Assessments for eligible program youth.
- Conduct Aftercare/Transitional Services Needs Assessments.
- Develop an Individualized Life Plan and life skills education plan for each youth.
- Arrange and provide groups, field trips, community projects, special events, and other opportunities.
- Track grant-required outcomes and report on them every 6 months.
- Provide targeted Aftercare services for individuals over 18 who are receiving follow-up services.
- Maintain program documentation on assessments, training, and services specific to the youth; maintain files, youth applications, assessments, and life skills education plans; conduct ILP reviews/staffings in the absence of the Director of Independent Living.
- Provide life skills coaching and support, including activities such as budgeting, housing search, and job applications.

Minimum of a Bachelor’s degree in social/human services;
Master’s degree is a plus;
Minimum 1 year experience in child welfare and human services; knowledge & experience with ESE youth and Independent Living preferred. Ability to work flexible hours. The program’s general operation hours are Monday through Friday from 8am to 8pm with weekends, as needed. Position is a 40-hour work week with hours varying on client needs and with weekly productivity expectations.
