$56.00/hr – $87.00/hr

As an Advanced Practice Provider in Primary Care, you will work with a team of healthcare professionals committed to providing quality care and an excellent patient experience. In this role you will:

• Work a 30‑hour week with Monday, Tuesday & Friday as your set schedule.
• Have approximately 27 patient‑facing contact hours and 3 hours of administrative time per week.
• Practice at Fort Lupton Walk‑In Clinic near Platte Valley Medical Center in Fort Lupton, Colorado.
• Have no on‑call responsibilities.
• Be eligible for full‑time benefit incentives, relocation if eligible, and a starting bonus.

• APRN or PA‑C degree from an accredited program.
• 2+ years of walk‑in or urgent care provider experience.
• Board certification or eligibility in Family Medicine/Primary Care (All Ages).
• Active, unobstructed Colorado medical license, or ability to obtain one.
• Full prescriptive authority and an active DEA, or ability to obtain one.
• Ability to successfully complete Intermountain Health’s credentialing process.
• Ability to navigate EPIC – our electronic health record.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. $56.76 – $87.59.
