Transition Coordinator - Assistant Head Nurse

As an Assistant Head Nurse within Transition Services, you'll blend clinical expertise with patient coordination in Fort McMurray. This role is ideal for those skilled in navigating complex health care transitions.

Key Responsibilities:

• Oversee staff assignments and optimize patient flow

• Provide frontline nursing care when needed

• Act as a clinical resource and advocate for nursing staff

• Coordinate quality patient care with interprofessional teams

• Manage operational priorities in care delivery

Requirements:

• Accredited nursing education program completion

• Active or eligible registration with CRNA

• Five years' experience in nursing

• Expertise in Patient and Family Centered Care (PFCC)

• Skilled in managing complex discharges
