Social Worker in Health Care Team Fort McKay

Fort McKay First Nation seeks a compassionate Social Worker to enhance family support services in a full-time role. Focus on implementing evidence-based treatments for improved client outcomes.
You'll collaborate closely with the Community Health & Wellness team to develop and monitor family-centered care plans. By assessing client needs with effective strategies, you'll be pivotal in navigating social and emotional challenges while ensuring access to vital community resources.

Your role will involve facilitating educational groups and documenting client interactions to ensure continuous improvement in care.

Key Responsibilities:

• Develop and customize family-centered behavioral care plans

• Regularly adjust care plans based on assessments

• Support navigation of community services for families

• Record client progress accurately and adhere to guidelines

• Participate in clinical collaboration for care enhancement
Requirements:

• Bachelor of Social Work Degree required

• Registration with the Alberta College of Social Workers

• 1-2 years of relevant experience in social work

• Experience in a First Nations community is a plus

• Clean criminal record and vulnerable sector check required
