Easter Seals Alberta seeks a dedicated Social Worker for the Grande Prairie or Fort Mac area. In this essential role, you will enhance community engagement and support individuals with disabilities through effective service navigation.

As a Social Worker, you will oversee program success while delivering essential support and resources tailored to community needs. This position demands strong interpersonal skills and a solid understanding of Alberta's disability support systems, particularly PDD, FSCD, and FMS. You will empower clients, families, and professionals, guide them through complex systems, and develop personalized service plans.

Key Responsibilities:

• Provide accurate information on disability supports and services

• Assist with needs-based assessments and service plans

• Build community partnerships for improved service delivery

• Facilitate training and learning opportunities for clients

• Monitor program performance and implement improvements

Requirements:

• Post-secondary education in Social Work

• 2-3 years' experience in case management
