Position Summary: The Care Manager is responsible for the development, maintenance, and implementation of the standards of nursing practice and objectives as it relates to the Minimum Data Set requirements, and with the established policies and procedures of the governing body in compliance with federal, state and local regulations. The Care Manager is responsible for optimal reimbursement, proper documentation/diagnoses to justify the reason for a skilled service and reimbursement request.
EssentialFunctions
- Coordinates/manages and works with clinical staff/therapy for proper documentation/care, programs and optimal reimbursement throughout the stay of a skilled resident
- Researches all pertinent medical information/diagnoses for optimal reimbursement
- Meets regulations for timely certifications/re-certifications, and signing of all orders
- Meets all requirements in working with Managed Care organizations
- Daily meetings with therapy and line staff on progress of skilled residents
- Monitors nursing skilled documentation and provides continued education as needed
- Participates in the development and implementation of the resident assessment process, following the Minimum Data Set requirements of federal, state and local regulations in accordance with Wellmore policies and procedures
- Responsible for proper ADL documentation and continued education/training
- Audits and analyzes the MDS documents for accuracy and appropriate completion dates
- Timely submissions of all MDS’s
- Oversees and schedules resident care planning meetings
- Assistance on floor and on-call/manager on duty as needed
- Participates in and attends all required in-service training and education sessions as scheduled

- Very active position requiring frequent standing, walking, bending, kneeling, stooping, and climbing
- Requires lifting or carrying weight up to 50 lbs. and pushing up to 250 lbs. independently
- Must use proper body mechanics to assist residents in daily living activities
- Regularly requires long hours and frequent night and weekend work
Travel:
No travel is expected, although occasional travel may be required for training sessions, continuing education opportunities, emergency situations, and other company functions.
- Four‑year college or university degree desired
- Current state licensure as a Registered Nurse
- Training in Medical Records regulations desired
