The Community Specialist creates individualized (1:1) or small group community-based services and supports, based on each participant’s goals, competencies, choices, and priorities as identified on the Individual Support Plan. The specialist mentors and facilitates independence and integration into the community by strengthening social interaction and community connectedness.

• Remain knowledgeable in and support the development of Individual Support Plans in collaboration with the individual, family, director, support coordinating agencies, and funders.
• Provide community living/integration supports, such as routine household tasks, activities of daily living related to community integration, personal hygiene, shopping, money management, medication management, socialization, relationship building, leisure choices, and participation in community activities.
• Provide or coordinate transportation to all activities.
• Provide direct supervision, training, care, and positive interaction with adults one-on-one or in small groups.
• Set up and help individuals use assistive/adaptive technology, including communication systems (with help of the ATRC).
• Use an integrated teaching approach to embed opportunities to improve social, communication, cognitive, and motor skills during community integration routines and activities.
• Adapt learning activities to support diverse abilities and learning styles, provide assistance and offer/respect choices.
• Engage individuals in physical activity relative to the community experience to build strength, endurance, and range of motion.
• Shape development of appropriate behavior and social interaction skills by encouraging communication and positive interactions with others, modeling positive social skills, redirecting, and purposefully mentoring development of personal/social responsibility.
• Provide opportunities for reflection after community activities to support active learning.
• Prepare and complete required documentation, including time/attendance records, contact notes for each date of service, a detailed monthly summary note documenting progress in achievement of individual objectives, incident reports, etc., with accuracy and timeliness.
• Provide care services required during community-based services and supports to address basic life needs, including physical handling of individuals, wheelchairs for transfers, positioning, personal hygiene, toileting, dressing, emergency response, and other tasks as needed.
• Provide direct supervision and positive interaction with individuals during all personal care services.
• Assist individuals with any type of personal care needed, encouraging independent participation in all personal care activities to foster increasing levels of independence.
• Participate in medication administration training as needed and as directed; administer medication as required by policy and accepted practices.
• Seek medical attention for individuals exhibiting signs of illness or injury; administer first aid or CPR as needed.
• Communicate pertinent information regarding each individual’s services, care, and well-being to the assistant director.

• Bachelor’s degree in any field from an accredited college or university with one year of experience in the field of intellectual or developmental disabilities.
• High school diploma and five years of experience in the field of intellectual or developmental disabilities as an alternative to a degree in human services education.
