Position: Palliative Medicine Physician (Advanced Heart Failure Support)

Overview

LPG Palliative Care in coordination with the Lee Health Heart Institute is seeking a physician with fellowship training and BE/BC in Hospice and Palliative Care Medicine to join their busy team.

Currently, the team consists of 4 physicians and 13 APRNs. This exclusive physician group is team-oriented and quality-focused and is currently seeking physicians interested in providing a mix of both inpatient and outpatient care. Inpatient care will be provided at the hospital setting with potential SNF opportunities as well.

Weekly call rotation is by phone 1:4 - 1:6.

Candidates for this position will be interested in doing coordinated care, medication & chronic symptom management, end of life management, and goals of care discussions. Our outpatient care is currently being redesigned for full integration into some of our high-utilization clinics (specifically oncology and advanced heart failure). This is a great opportunity for someone with an interest in development, program growth and out of the box thinking with a high focus on exceptional patient care to join the team and assist with this strategic redesign process.

Responsibilities
  • Provide a mix of inpatient and outpatient hospice and palliative care.
  • Inpatient care at the hospital setting with potential skilled nursing facility opportunities.
  • Participate in coordinated care, medication and chronic symptom management, end-of-life care, and goals of care discussions.
  • Support the outpatient redesign to integrate into high-utilization clinics (oncology and advanced heart failure).
  • Engage in development and growth of programs with a focus on exceptional patient care.
Qualifications
  • MD/DO with fellowship training in Hospice and Palliative Medicine (BE/BC).
  • Active Florida License, or the ability to obtain this prior to start date.
  • Active, Unrestricted DEA.
  • Current BLS Credentials through the American Heart Association.
