Ambassador Healthcare at College Park is seeking a compassionate and motivated Social Service Assistant to join our team. In this role, you will support the planning, organization, and implementation of social services to ensure residents receive safe, comprehensive care during their stay and smooth transitions upon discharge.

• Conduct resident and family interviews to gather social history.
• Assist with discharge planning and coordinate with community resources.
• Support the development and implementation of individualized social care plans.
• Participate in resident assessments and departmental programs.
• Report occupational exposures in accordance with facility policy.
• Take part in emergency drills and fulfill duties under the facility’s emergency plan.
• Stay informed on best practices and integrate them into social service programs.

• Minimum 1 year of social work experience in a healthcare setting.
• Medicaid experience in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) preferred.
• Strong customer service background, particularly with elderly populations.
• Currently pursuing a degree in social work or a related field preferred.
• Previous social service experience and education a plus.
