We are seeking a caring and reliable Full-Time Licensed Practical Nurse (LPN) to join our home health care team. As an LPN, you will provide compassionate, patient-centered care to individuals in their homes under the supervision of a Registered Nurse (RN). You will play a critical role in helping patients manage their health, recover from illness or surgery, and improve their quality of life while staying in a familiar and comfortable environment.

Key Responsibilities:

Deliver direct patient care, including monitoring vital signs, wound care, and administering medications.

Follow established care plans under the supervision of an RN or physician.

Assist patients with daily living activities, including mobility, hygiene, and nutrition.

Educate patients and their families about home care practices, medication administration, and health management.

Collaborate with healthcare providers and the home health team to ensure the best possible patient outcomes.

Document patient progress, health status, and care provided in the electronic medical record (EMR) system.

Monitor for any changes in a patient’s condition and promptly report to the RN or physician.

Adhere to agency policies, procedures, and state/federal regulations to ensure compliance and quality care.

Qualifications:

Active and unrestricted Licensed Practical Nurse (LPN) license in Florida.

Minimum of 1 year of clinical experience, preferably in home health or a similar setting.

Strong knowledge of home health care practices and state regulations.

Proficiency in electronic medical record (EMR) systems.

Excellent communication and interpersonal skills.

Ability to work independently, manage multiple patients, and prioritize tasks effectively.

Compassionate, patient-focused, and detail-oriented.

Valid driver’s license and reliable transportation.
