The Palms Home Care is seeking an experienced home health RN for a full-time, salaried Weekend Baylor position. This role provides skilled nursing care Friday through Monday, with a productivity expectation of 32 points per week.
ResponsibilitiesPerform skilled nursing visits and comprehensive assessments, including OASIS start-of-care, resumption-of-care, recertification, and discharge assessments.
Provide medication management, wound care, and patient and caregiver education.
Develop and update individualized plans of care in collaboration with physicians and the clinical team.
Monitor changes in patient condition and promptly communicate concerns.
Coordinate care with patients, families, and other healthcare professionals.
Complete accurate, timely documentation in Well Sky/Kinnser.
Meet the weekly productivity requirement of 32 points.
Active Florida RN license and current CPR certification.
Previous home health nursing experience and OASIS proficiency.
Strong clinical judgment, communication, and time-management skills.
Ability to work independently within the assigned territory.
Valid driver’s license and reliable transportation.
Well Sky/Kinnser experience preferred.
